Initial Position of a Primitive

Ok, this is probably a very basic question but I seem to be missing something fundamental.

When I create a primitive I only seem to be able to create it in a 2D plane, either XY, XZ or YZ depending on my view. Is this how it should work and what is the best way of working with this system if it is?

Also, the origin is always taken from the last primitive - is there a way of always using the world origin?
