I try filleting the intersection between two overlapping primitives by using a boolean union operation and having it auto select the resulting edges. Then I extrude the edges, but the result is nearly always quite messy.

Well the way you describe what you did, there's no polygonal modeler that'll give you any good result like that... You won't get anything good making booleans and filleting them that way. Polygonal and subdivision modelling is totally differents from Nurbs modelling, which what you have in CAD packages.
To make good fillets in subdivision modelling, you have to cope with the smoothing... That'll come with time...
